TIMELY SURGERY FOR A RARE CASE: LEFT PARADUODENAL INTERNAL HERNIA

Recently, Tam Tri Nha Trang General Hospital received a 50-year-old female patient, Ms. N.L.Q. (residing in the eastern area of Ninh Hoa), who was admitted with severe abdominal pain and persistent vomiting lasting for two days.

Immediately upon admission, doctors conducted a clinical examination, abdominal ultrasound, abdominal CT scan, and specialized diagnostic tests. The diagnostic results confirmed that the patient was suffering from intestinal obstruction caused by a left paraduodenal internal hernia—a very rare surgical condition. The patient was promptly scheduled for laparoscopic surgery to resolve the cause of the obstruction.

During the procedure, the herniated bowel loops were successfully released, and the hernia defect was closed via minimally invasive laparoscopy. Thanks to the timely intervention, the patient recovered well, her vital signs remained stable, and she has been discharged from the hospital.

A left paraduodenal internal hernia occurs when a segment of the small intestine herniates into an abnormal peritoneal recess in the left upper abdomen. When the bowel loops become trapped at the neck of the hernia sac, the intestinal obstruction can rapidly progress to strangulation, compromising blood circulation to the intestine and leading to tissue necrosis.

The danger of this condition lies in the fact that patients usually do not have a visible external hernia bulge on the abdominal wall. Identifying the disease requires close collaboration between clinical examination, diagnostic imaging, and expert surgical evaluation.

Healthcare experts advise individuals to visit a medical facility immediately upon experiencing symptoms such as crampy or progressively worsening abdominal pain, vomiting, abdominal distension, or the inability to pass gas or stool. In particular, recurrent abdominal pain of unknown origin should be thoroughly evaluated to detect potential underlying medical conditions.
