Mr. H, a 65-year-old patient, visited the Gastrointestinal Endoscopy Unit at Tam Tri Nha Trang General Hospital for an endoscopic examination. During the procedure, doctors identified multiple colorectal polyps and a laterally spreading tumor (LST) measuring approximately 30 × 35 mm.

Combined assessment using Narrow-Band Imaging (NBI) revealed increased vascularity within the lesion, suggesting a high risk of malignant progression. This case required definitive treatment rather than monitoring alone.

Given the lesion’s large size and high risk, the endoscopy team performed Endoscopic Submucosal Dissection (ESD).

ESD is an advanced endoscopic technique that allows doctors to dissect and remove the entire lesion through an endoscope while minimizing damage to the surrounding healthy tissue.

Outcome: The lesion was completely removed while preserving the structure and function of the digestive tract, eliminating the need for bowel resection. The patient gradually recovered, experienced no abdominal pain or bleeding, and was discharged just 24 hours after the procedure.

For appropriately selected cases, this minimally invasive technique can reduce post-procedure pain, promote faster recovery, and shorten hospital stays compared with surgical bowel resection.

Notably, histopathological examination revealed a tubular adenoma with low-grade dysplasia. This type of lesion needs to be detected and treated early to help prevent potential progression.

This case once again highlights the importance of colorectal cancer screening and early detection of abnormal lesions. When lesions are identified and treated promptly, patients may have the opportunity to receive minimally invasive treatment rather than undergo more extensive surgery if the disease progresses.

The implementation of ESD at Tam Tri Nha Trang General Hospital expands the range of advanced treatment options available for patients with precancerous lesions and selected early-stage gastrointestinal cancers when clinically indicated.
