“Silently Accumulating in the Brain for Months, a Chronic Brain Hematoma Nearly Cost a 76-Year-Old Man His Life"
Not all brain disorders begin with sudden, severe symptoms. Some hematomas can silently enlarge over the course of months. By the time patients become drowsy, develop weakness in their limbs, or lose consciousness, the brain may already be under severe compression.
Recently, Tam Tri Nha Trang General Hospital admitted a 76-year-old man in a drowsy state with decreased alertness and weakness on the right side of his body. According to his family, he had experienced poor balance and increasing difficulty walking for an extended period. However, his condition suddenly worsened on the morning of admission, prompting them to bring him to the Emergency Department.
Upon examination, the patient was found to be drowsy with a Glasgow Coma Scale (GCS) score of 13 and right-sided hemiplegia. CT and MRI scans revealed a 3 cm chronic subdural hematoma over the left cerebral hemisphere, causing a 2 cm midline shift and subfalcine brain herniation - a neurosurgical emergency requiring immediate surgery to relieve the life-threatening pressure on the brain.
The hospital's Neurosurgery team promptly performed emergency surgery to evacuate the hematoma and decompress the brain.
Just 4 hours after surgery, the patient was fully awake, responsive, and improved to a GCS score of 15, with significant recovery of motor function. After 24 hours, his headache had subsided, and he was able to walk again.
By 48 hours, the drainage tube was removed, he was fully conscious, and muscle strength in all four limbs had returned to 5/5. A follow-up CT scan confirmed complete removal of the hematoma. The patient made a good recovery and was discharged after 5 days of treatment.
Chronic subdural hematoma is a common condition among older adults. Because it develops gradually, its symptoms are often mistaken for normal signs of aging, such as slower walking, poor balance, forgetfulness, or excessive sleepiness. If persistent headaches, drowsiness, limb weakness, or changes in consciousness occur, patients should seek medical attention as soon as possible to prevent serious complications caused by brain compression.
