PERSISTENT SPINAL PAIN LEADS TO SPINAL TUBERCULOSIS DIAGNOSIS THROUGH TRANSPEDICULAR SPINAL BIOPSY

After suffering from persistent thoracic back pain for an extended period without improvement despite medication, a 29-year-old man, Mr. T., visited Tam Tri Nha Trang General Hospital to determine the underlying cause.

Following a clinical examination, doctors ordered an MRI scan of his spine. The imaging revealed inflammation of the T10 vertebral body accompanied by a vertebral abscess, suggesting a serious spinal lesion. However, MRI findings alone were not sufficient to determine the exact cause. The medical team considered several possibilities, including pyogenic spondylitis (bacterial spinal infection), spinal tuberculosis, and spinal tumors.

To establish a definitive diagnosis, the hospital's Neurosurgery and Spine Surgery team decided to perform a transpedicular spinal biopsy. This technique uses specialized instruments to access the vertebral body through the pedicle, allowing tissue samples to be collected from the lesion for histopathological examination and other specialized laboratory tests.

The biopsy results confirmed that the patient had spinal tuberculosis.

Immediately after the diagnosis was confirmed, the patient received comprehensive counseling regarding his condition, and an appropriate treatment plan was developed. Early and accurate diagnosis enabled targeted treatment, helping prevent further spinal damage and reduce the risk of neurological complications.

A transpedicular spinal biopsy is an important diagnostic technique that enables doctors to obtain tissue samples directly from spinal lesions to accurately identify the underlying cause of conditions such as spondylitis, spinal tuberculosis, or spinal tumors. In addition to MRI and CT imaging, this procedure plays a crucial role in establishing an accurate diagnosis and guiding the most effective treatment for each patient.
